With its headquarters in Tempe, Arizona, OnePoint delivers a unique array of pharmacy solutions, including local and mail-order dispensing, clinical support, and tailored disease-state management. Serving clients nationwide, OnePoint Patient Care has over 100 pharmacy locations and a network of 55,000 contracted pharmacies, employing more than 200 people to provide critical services to hospice facilities and in-home patients.

Incident Overview
On August 8, 2024, OnePoint Patient Care detected unusual activity in its systems, leading to an investigation into a potential data breach. By August 15, 2024, it was confirmed that an unauthorized party may have accessed sensitive information during the period from August 6 to August 8, 2024. This data may include:
- Full names
- Social Security numbers
- Residential details
- Medical information (such as medical record numbers, diagnosis, and prescription details)
On October 14, 2024, OnePoint Patient Care publicly announced the breach on its website and began notifying impacted individuals by mail. To assist those affected, OnePoint is offering credit monitoring services along with details about the information potentially exposed.
